Residential Mental Health and I/DD Services

Residents receive supervision and support 24 hours a day, including awake overnight staffing. Team members are available to help with daily routines, safety needs, behavioral and medical concerns, emergency response, and coordination with authorized providers and supports.

Before services begin, qualified staff review the applicant's diagnosis, medical status, behavioral history, functional and evacuation needs, current Person-Centered Plan or treatment plan, and authorization.

Following admission, the resident and authorized participants help develop an individualized plan based on strengths, needs, preferences, and goals.

Support may include activities of daily living, personal hygiene and grooming, healthy routines,

housekeeping habits, organization, and other practical skills. Assistance reflects the resident's abilities, preferences, privacy, and plan.

Trained and authorized staff support medication routines according to current orders and agency procedures. This may include administration, documentation, secure storage, monitoring, provider coordination, and appointment follow-up when included in the resident's plan.

Staff use person-centered communication, positive behavioral support, and approved de-escalation practices. Support is provided in the least restrictive manner appropriate to the resident's needs, safety, rights, and plan.

Residents are encouraged to build healthy relationships and participate in community life. Opportunities may include events, recreation, hobbies, clubs, peer activities, spiritual practices, outdoor time, and outings based on each person's interests, abilities, and goals.

When included in a resident's plan, transportation is coordinated for authorized appointments,

activities, and other needs with attention to safety, comfort, scheduling, emergency information, and confidentiality.

Residents have clean, comfortable living spaces and common areas for relaxation and connection. Privacy is respected, belongings are safeguarded, and rooms may be personalized within safety guidelines.
